REQUIREMENTS TO U.S. PASSPORT NAME CHANGE
- If the change is within one year after issuance, please fill out form CLICK HERE
- When you receive completed form please sign
- If the passport was issued more than one year ago, please fill out CLICK HERE When you receive completed form please sign.
- Two passport type photos. We recommend that you have these pictures taken by a facility familiar with passport photo requirements such as Walgreens, CVS, etc.
- Proof of Identity (A Valid Driving License, Government Issue Photo I.D)
- Original or certified (NOT notarized) copy of the document showing the name change (example: Marriage License, Court Order, Divorce Decree, etc.)
- An International Passport Visa letter of authorization
- Completed Credit Card Authorization form
