Boliva

Tourist Visa
  • A valid U.S. Passport is required. (must be valid at least 6 months beyond date of arrival, and contain at least one blank visa page.)
  • 1 Visa application form fully completed and signed.
  • 1 Passport type Photograph.
  • Copy of Hotel Confirmation, or letter of invitation from friends or relatives in Bolivia.
  • Copy of Round Trip Airline Ticket or copy of travel agency/airline Itinerary.
  • Copy of Bank Statement (copy of Credit Card also accepted)
  • International Certificate of Vaccination for Yellow Fever is required (except for minors age under 1 year).
  • Completed Credit Card Authorization form  
  • US minors traveling alone, must hold a travel authorization from absent parent(s)/guardian(s). Translation into Spanish recommended.

Business Visa
  • Submit U.S. Passport /must have at least 6 months validity remaining and contain at least one blank visa page
  • 1 Visa application form, completed and signed
  • 1 Recent Passport-Type Photograph
  • Copy of Itinerary
  • International Certificate of Vaccination for Yellow Fever is required
  • Completed Credit Card Authorization form  
  • A business letter of responsibility is required, to be typewritten on your company letterhead and addressed to: 

Embassy of Bolivia, Visa Section
  • The letter must explain all of the following
  • Purpose of the trip
  • Name and address of company to be visited
  • Guarantee of financial responsibility and return transportation

Processing Instructions
